PHP Application To Monitor Windows Server and Linux

I need some information/reference how to create a PHP application to monitor below stuffs on windows servers -

Disk Space

Uptime

RAM

CPU Usage

User Login History

Last Reboot etc.

Any info on this would help. TIA :)
